As a dental practice, Millennium Dental is required by the Health Insurance Portability and Accountability Act (HIPAA) to protect the privacy of your health information and to give you notice of our legal duties and privacy practices.
Our full Notice of Privacy Practices explains how we may use and share your health information, your rights to see and get copies of your records, to request corrections and to receive an accounting of disclosures, and how to file a complaint.
